MEMBERSHIP DECLARATION
By completing the membership form attached or on the website www.silksocialclub.com, I hereby apply for membership of Silk Social Club. In consideration of my application for membership being accepted I acknowledge and agree that:
1. In this membership declaration:
- Silk Social Club is not a dating service or swinger’s club.
- Silk Social Club has the right to revoke your membership for any reasons such as/and not limited to the following: misconduct/inappropriate behavior, and deliquent account history (30 days or more).
- All membership dues are non-refundable.
- Your membership status must be active in order to participate in any events. Your membership services are not transferable to any other person or entity.
CANCELLATION POLICIES AND PROCEDURES
- You may cancel this Agreement after the initial term of one year with 30 days notice.
- Cancellation During Auto-Renewal Term: During the auto-renewal term of your membership, you may cancel your membership upon 30-day advance written notice to Silk Social Club.
- All requests for cancellation of membership must be submitted in writing to Silk Social Club. You are responsible for all membership fees incurred until you cancel your membership in accordance with the terms of this Agreement.
- We reserve the right to terminate or deny re-enrollment for an indeterminate amount of time if a customer has an unsatisfactory payment history.
OTHER PROVISIONS
LATE CHARGE: If all or part of any scheduled payment is more than 5 days late, you will be charged a late fee of $10.00.
NOTICE TO CUSTOMER
You are entitled to a copy of this contract at the time you sign it. If you choose to cancel this contract, you must send a signed and dated written notice of cancellation by mail, return receipt requested: Silk Social Club, PO Box 2162, Clinton, MD 20735.
